Thank you for choosing us as your healthcare provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered part of your treatment. We accept cash, checks, Visa/Mastercard.
Questions About Your Bill? - If you have a question about your bill, you should contact the customer service number listed on the bill.
Insurance
We accept most major insurance plans, including government payers. For insurance plans where we are a participating providers - all co-pays and deductibles are due at the time of service. In the event that your insurance coverage changes to a plan where we are not a participating provider, the balance is your responsibility.
Referrals
Various insurance companies require an inter-office referral. Please bring the referral with you if our office has not received it in the days preceding the appointment. If a co-payment is due for your visit, the co-payment is due the day of your appointment.
