After Hours & Emergencies
If you are needing to contact the office after hours, you can call our main number and leave a voice mail. If you are experiencing a medical emergency or need immediate assistance, our physicians recommend that you Dial 9-1-1.
Prescription Refills
When needing a refill on your prescription, contact your pharmacy and they will send our office a refill request. It takes up to 10 business days to refill the prescription.
Medical Records
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
