Office Hours & Appointments
To schedule an appointment call (303) 481-0030 during regular business hours. Our office is open from 8:00am - 4:30pm Monday - Friday. We are closed between 12 noon and 1:00pm for lunch.
Dr. Simon Oh sees patients at the Centennial Medical Plaza each week. Please call our main office number to find out the days and times that he will be available.
Visit Requirements
If you have had any testing (MRI, X-Rays, CT scan and/or an EMG) done, please bring a copy of the report along with the actual study if applicable. We may have to reschedule your appointment if you come without the studies. You may pick up these films at the imaging center where the test was performed. Please call the facility 24 hours in advance to have your films or compact discs ready.
New Patients
Please arrive 30 minutes prior to your scheduled appointment time to complete the necessary registration forms.
Cancellations
Please call 24 hours in advance if you must cancel an appointment so we can open the schedule for other patients who wish to be seen.
After Hours & Emergencies
If you are needing to contact the office after hours, you can call our main number and leave a voice mail. If you are experiencing a medical emergency or need immediate assistance, our physicians recommend that you Dial 911.
